ABSTRACT

OBJECTIVE: Present a giant adrenal cyst (AC) case treated with minimally invasive resection and to perform a narrative literature review available. MATERIAL AND METHODS: A 54 year-old male presents with a left retroperitoneal slow growing mass, no symptoms, with a complex AC evidenced by previous images and mass biopsy, with suspected renal infiltration. A transabdominal laparoscopic resection is indicated. RESULTS: A complex hemorrhagic 9 cm diameter AC was found, with adhesions to left Gerota's fascia. Complete resection of the AC was achieved through minimally invasive approach. The patient had an uneventful clinical recovery and was discharged on the second postoperative day. On 19th month of follow-up is completely asymptomatic. Even though the AC are benign lesions, the symptomatic giant AC, with fast growing ratio, and/or hemorrhagic conversion could be resected though laparoscopic adrenalectomy, with no increased morbidity or mortality. DISCUSSION: The laparoscopic approach for giant non-functional AC should be considered as the standard of care. More evidence is required in terms of surgical approach outcomes to define clear recommendations. CONCLUSION: This report adds to the actual evidence in terms of minimally invasive approach for hemorrhagic giant AC.

ABSTRACT

Introduction: Combined liver and kidney transplantation (CLK) has been shown to be a good alternative for patients with concomitant diagnosis of chronic kidney disease (CKD) and end-stage liver disease. Some studies have also shown immunological benefits from combined transplantation with decreased rates of kidney graft rejection. Objective: The objective of this study was to describe the indications and clinical outcomes of CLK transplant recipients in a highly complex hospital. Materials and Methods: CLK transplant patients were selected from the institutional transplant registry (Trenal) from 2000 to 2011. Demographic and clinical characteristics were described and survival of patients and grafts were estimated with the Kaplan-Meier method. Results: Over a period of 11 years, 16 CLK transplants were performed. This was 1.51% of kidney transplants and 3.48% of liver transplants done in the institution during this period. Most recipients were male (10/16). The median age was 56.5 years. The median MELD was 17 (IQR: 12.5 to 20.5, range: 8 to 24). The most frequent diagnosis was liver cirrhosis due to NASH (4/16). All patients had been diagnosed with CKD: four cases were secondary to diabetes mellitus. The most common indications for transplants were difficult to manage ascites, recurrent encephalopathy and malnutrition. The average liver cold ischemia time was 7.3 hours, and the average kidney cold ischemia time 9.6 hours. The five-year liver graft survival rate was 87.5%, and the five-year kidney graft survival rate was 67.3%. Four patients died: two as the result of sepsis and two as the result of malignancy. Conclusion: CLK transplantation results at the Fundación Valle del Lili have been satisfactory and comparable to those reported by other transplant centers.

ABSTRACT

The aim of this trial was to study the effect of different grazing intervals on productive characteristics of persian clover and ryegrass, in consortium and managed under rotational stocking during the years 2009 and 2010. Treatments were composed of four grazing intervals (time to onset of 2.5; 3.5; 4.5 and 5.5 leaves in persian clover plants) allocated in completely randomized blocks, with six replications. Leaf area index (LAI), leaf area, pasture and plants height, length of tillers/branches, rate of dry matter accumulation and dry matter yield were evaluated. The leaf area index of the canopy and the persian clover was higher in the interval of 5.5 leaves in years 2009 and 2010. In 2009, higher values of leaf area were obtained in intervals of 3.5 and 4.5 leaves and there no was effect of grazing intervals on this variable in 2010. Higher grazing intervals provided greater pasture height and plant height and also greater tiller and branch length. The dry matter production of persian clover was higher in 2010, while for ryegrass highest yields were found in 2009. Total dry matter yield was higher in the interval of 5.5 leaves. The rate of dry matter accumulation was higher in the interval of 5.5 leaves in 2009 and in the interval of 2.5 leaves in 2010. The productive characteristics of a of mixture persian clover and ryegrass are modified by the grazing intervals. It is recommended intervals up to 3.5 of appeared leaves with a mixture of persian clover and ryegrass, which provides lower elongation of stems associated with a lower presence of dead material, which probably improves the quality of the harvested forage.

ABSTRACT

Introduction: Surgical treatment of liver metastases is broadly accepted for colorectal and neuroendocrine tumors. However, the approach to liver metastases of other origins is not well established. The objective is the evaluation of factors that might be associated with better or worse perioperative outcomes (morbidity and mortality) and survival of patients with non-colorectal, non-neuroendocrine liver metastases (NCNNLM) treated surgically in a second level hospital. Material and Methods: A prospective study of patients with NCNNLM treated surgically or with radiofrequency was completed in San Cecilio University Hospital of Granada, between March 2003 and June 2013. The statistical analysis was made with IBM-SPSS version 21. Statistical significance was set at p ≤ 0.05. Survival data were analyzed by Kaplan-Meier's method. Results: Twenty-two patients with 26 surgeries were analyzed, 19 liver resections, 6 radiofrequency treatments and 1 combined case with surgery and radiofrequency. Operatory mortality was 0 percent and overage mortality was 23.1 percent. Univariate analysis only showed statistical significance in two of the factors, presence of positive lymphatic nodes during the surgery of the primary tumor and presence of extra-hepatic disease. Accumulated survival was 81 percent, 66 percent and 52 percent at 1, 2 and 3 years respectively. Conclusions: Despite the fact of treating together in the same group several different tumors, the surgical treatment of NCNNLM (with or without radiofrequency) has an acceptable survival rate. Thus, they can be treated with safety when we use a multi-disciplinary approach to these patients.

ABSTRACT

Objective. To establish the relationship between ultrasonographic measurements of muscle and fat in back and hip of Brahman cattle fed for cattle shows and their association with serum levels of total cholesterol (CT), high density lipoprotein (HDL), low density lipoprotein (LDL), triglyceride (TG), β-hydroxybutirate (β-OHB), and free thyroxin (T4). Materials and methods. One hundred and eighty animals were purposively selected. The age varied between 9 and 24 months. Ultrasonographic measurements from the loin eye area (AOL), depth of gluteus medius (PMGM), backfat thickness (EGD), and hip fat thickness (EGA) from each animal were recorded. Blood serum samples were collected to determine total cholesterol (CT), lipoproteins (HDL and LDL), triglycerides (TG), and thyroxin (T4). Correlation and linear regression analyses were conducted. Results. Gender and weight were related to muscle and fat coverage measurements in the backs and hips of the animals. Differences in serum metabolites were also related to gender and weight. Although there was a significant correlation between the ultrasonographic measurements and serum metabolites, the associations were weak. Conclusions. The relationship between ultrasonographic measurements of back and hip with serum lipid metabolites is weak, which indicates that those metabolites are not suitable to predict the productive performance of Brahman cattle fed for cattle shows.

ABSTRACT

This study describes, from a qualitative approach, the experiences that some women have in Bogota, Colombia with the treatment and management of premenstrual syndrome or menstrual disorders, giving special attention to the socio-demographic and cultural contexts in which they are embedded. In order to develop this study, ten women diagnosed with PMS, dysmenorrhea, amenorrhea and metrorrhagia were given semi-structured, in-depth interviews. It can recognised that these women's experiences were subject to their beliefs and cultural understanding, their support network, diagnosis, symptoms and its impairments in their daily lives, opportune treatment, and legitimization of their illness and effectiveness of the health care. Conventional treatments excel as a pillar for the management of menstrual discomfort for women regardless of diagnosis and whether or not they are receiving or not medical treatment at the same time.

ABSTRACT

Brain metastases are observed in approximately 1 percent of patients with differentiated thyroid cancer. We report a 56 years old woman subjected to a thyroidectomy due to a follicular thyroid cancer with lymph node involvement. The patient also received 79 mCi of radioiodine and levothyroxine was started. One year later a local relapse and lung dissemination was found. The patient was subjected to a surgical neck exploration and radioiodine in a dose of 250 mCi was given again. One year after this episode, a new local relapse was diagnosed and lung metastases increased in size and number. The patient presented a left hemiparesis and a brain CAT scan and magnetic resonance imaging showed multiple metastatic lesions. The patient was considered intractable and died eight months later.

ABSTRACT

This work evaluated the bactericidal activity and determinated the Minimum Inhibitory Concentration (MIC) of ethanolic extract and essential oil from Rosmarinus officinalis L. leaves on microorganisms of interest in food industry: Escherichia coli, Staphylococcus aureus, Salmonella typhimurium, Shigella sonnei, Listeria monocytogenes, Pseudomonas aeruginosa, Bacillus cereus and Lactobacillus plantarum. The essential oil showed a broad spectrum of antimicrobial action for both Gram positive and Gram negative bacteria with MICs between 512 - 4096 ppm. The ethanolic extract showed antimicrobial activity against S. sonnei, S. typhimurium and L. monocytogenes with a MIC of 1024 ppm. Nisin was used as positive control and showed a strong growth inhibition of all bacteria tested with MICs between 2 and 1024 ppm. Our result shows that preservatives commonly used in the food industry have lower antimicrobial activity than those found in essential oil from R. officinalis L. leaves.

ABSTRACT

This cross-sectional descriptive study was carried out to know the prevalence of dengue and leptospirosis in the inhabitants of Carmen del Paraná (Itapúa). Through a house hold sampling, we applied a questionnaire and collected blood samples. The heads of the families were informed about the methodology, benefits and possible risks so that they could decide about the participation of their families in the study. The presence of anti-leptospira IgG antibodies was determined by ELISA method in 320 individuals (211women and 107 men) and anti-dengue virus antibodies in 399 individuals (255 women and 142 men). We found a dengue virus seroprevalence 25% (IC 95 %:20.7-29.4) without any differences between men and women. The prevalence of leptospirosis was24.1% (IC95 %:19.6-29.2), higher in women than in men (28.4 % vs. 15.9 %; p value <0.05). Dengue seroprevalence was higher in the age group older than 35 years old(36% vs. 17%, value of p <0.001), where as leptospirosis was more prevalent in those younger than 15 years old (48% vs 21.6%, p value <0.001). Eight percent of the individuals who were leptospira positive were also dengue positive. In this community, there is an important number of individuals who has a risk of developing hemorrhagicdengue. The coexistence of dengue and leptospirosis in the same ecological niche is important because both diseases presented similar clinical characteristics, implying a problem for the physician at the time of determining the appropriate diagnosis andtreatment. Therefore, it would be important to develop integral strategies of epidemiological surveillance, prevention and control of these diseases, based on the jointparticipation of healthcare personnel, authorities and community.
